Overview

Diabetes is a chronic disease that occurs either when the pancreas does not produce enough insulin or when the body cannot effectively use the insulin it produces. Insulin is a hormone that regulates blood glucose. Hyperglycaemia, also called raised blood glucose or raised blood sugar, is a common effect of uncontrolled diabetes and over time leads to serious damage to many of the body’s systems, especially the nerves and blood vessels.

In 2014, 8.5% of adults aged 18 years and older had diabetes. In 2019, diabetes was the direct cause of 1.5 million deaths and 48% of all deaths due to diabetes occurred before the age of 70 years. Another 460 000 kidney disease deaths were caused by diabetes, and raised blood glucose causes around 20% of cardiovascular deaths (1).

Between 2000 and 2019, there was a 3% increase in age-standardized mortality rates from diabetes. In lower-middle-income countries, the mortality rate due to diabetes increased 13%.

By contrast, the probability of dying from any one of the four main noncommunicable diseases (cardiovascular diseases, cancer, chronic respiratory diseases or diabetes) between the ages of 30 and 70 decreased by 22% globally between 2000 and 2019. 

Symptoms

Symptoms of diabetes may occur suddenly. In type 2 diabetes, the symptoms can be mild and may take many years to be noticed.

Symptoms of diabetes include:

  • feeling very thirsty
  • needing to urinate more often than usual
  • blurred vision
  • feeling tired
  • losing weight unintentionally

Over time, diabetes can damage blood vessels in the heart, eyes, kidneys and nerves.

People with diabetes have a higher risk of health problems including heart attack, stroke and kidney failure.

Diabetes can cause permanent vision loss by damaging blood vessels in the eyes.

Many people with diabetes develop problems with their feet from nerve damage and poor blood flow. This can cause foot ulcers and may lead to amputation.

Type 1 diabetes

Type 1 diabetes (previously known as insulin-dependent, juvenile or childhood-onset) is characterized by deficient insulin production and requires daily administration of insulin. In 2017 there were 9 million people with type 1 diabetes; the majority of them live in high-income countries. Neither its cause nor the means to prevent it are known.

Type 2 diabetes

Type 2 diabetes affects how your body uses sugar (glucose) for energy. It stops the body from using insulin properly, which can lead to high levels of blood sugar if not treated.

Over time, type 2 diabetes can cause serious damage to the body, especially nerves and blood vessels.

Type 2 diabetes is often preventable. Factors that contribute to developing type 2 diabetes include being overweight, not getting enough exercise, and genetics.

Early diagnosis is important to prevent the worst effects of type 2 diabetes. The best way to detect diabetes early is to get regular check-ups and blood tests with a healthcare provider.

Symptoms of type 2 diabetes can be mild. They may take several years to be noticed.  Symptoms may be similar to those of type 1 diabetes but are often less marked. As a result, the disease may be diagnosed several years after onset, after complications have already arisen.

More than 95% of people with diabetes have type 2 diabetes. Type 2 diabetes was formerly called non-insulin dependent, or adult onset. Until recently, this type of diabetes was seen only in adults but it is now also occurring increasingly frequently in children.

Gestational diabetes

Gestational diabetes is hyperglycaemia with blood glucose values above normal but below those diagnostic of diabetes. Gestational diabetes occurs during pregnancy.

Women with gestational diabetes are at an increased risk of complications during pregnancy and at delivery. These women and possibly their children are also at increased risk of type 2 diabetes in the future.

Gestational diabetes is diagnosed through prenatal screening, rather than through reported symptoms.

Impaired glucose tolerance and impaired fasting glycaemia

Impaired glucose tolerance (IGT) and impaired fasting glycaemia (IFG) are intermediate conditions in the transition between normality and diabetes. People with IGT or IFG are at high risk of progressing to type 2 diabetes, although this is not inevitable.

Prevention

Lifestyle changes are the best way to prevent or delay the onset of type 2 diabetes.

To help prevent type 2 diabetes and its complications, people should:

  • reach and keep a health body weight
  • stay physically active with at least 30 minutes of moderate exercise each day
  • eat a healthy diet and avoid sugar and saturated fat
  • not smoke tobacco.

Diagnosis and treatment

Early diagnosis can be accomplished through relatively inexpensive testing of blood glucose. People with type 1 diabetes need insulin injections for survival.

One of the most important ways to treat diabetes is to keep a healthy lifestyle.

Some people with type 2 diabetes will need to take medicines to help manage their blood sugar levels. These can include insulin injections or other medicines. Some examples include:

  • metformin
  • sulfonylureas
  • sodium-glucose co-transporters type 2 (SGLT-2) inhibitors.

Along with medicines to lower blood sugar, people with diabetes often need medications to lower their blood pressure and statins to reduce the risk of complications.

Additional medical care may be needed to treat the effects of diabetes:

  • foot care to treat ulcers
  • screening and treatment for kidney disease
  • eye exams to screen for retinopathy (which causes blindness).

WHO response

WHO aims to stimulate and support the adoption of effective measures for the surveillance, prevention and control of diabetes and its complications, particularly in low- and middle-income countries. To this end, WHO:

  • provides scientific guidelines for the prevention of major noncommunicable diseases including diabetes;
  • develops norms and standards for diabetes diagnosis and care;
  • builds awareness on the global epidemic of diabetes, marking World Diabetes Day (14 November); and
  • conducts surveillance of diabetes and its risk factors.

In April 2021 WHO launched the Global Diabetes Compact, a global initiative aiming for sustained improvements in diabetes prevention and care, with a particular focus on supporting low- and middle-income countries.

In May 2021, the World Health Assembly agreed a Resolution on strengthening prevention and control of diabetes. In May 2022 the World Health Assembly endorsed five global diabetes coverage and treatment targets to be achieved by 2030.

A C-peptide test measures the amount of C-peptide in the blood or urine. The pancreas releases C-peptide when it makes insulin. The test can help determine the type of diabetes you have or how well diabetes treatments are working. The test also can help diagnose pancreatic cancer, kidney failure, Cushing syndrome or Addison disease.

What is a C-peptide test?

A C-peptide test measures the amount of C-peptide in the blood or urine. It can help healthcare providers determine what type of diabetes a person has: Type 1 or Type 2. It also can reveal how well diabetes treatments are working. Another use for C-peptide is to determine if your pancreas is making insulin.

The test is sometimes called:

  • Connecting peptide insulin.
  • Insulin C-peptide.
  • Proinsulin C-peptide.

What is C-peptide?

C-peptide is a byproduct the pancreas releases into the body when it makes insulin. The pancreas produces about the same amount of insulin and C-peptide.

Insulin is a hormone that regulates the amount of sugar (glucose) in the blood. Glucose is the human body’s main source of energy. Insulin allows cells in the muscles, fat and liver to absorb glucose to be used as energy.

In people with Type 1 diabetes, the pancreas isn’t able to make insulin. They need insulin injections so the body can use glucose. People with Type 2 diabetes make insulin, but their bodies either don’t produce enough insulin or resist the effects of insulin. This test can tell the difference between insulin made by the body (endogenous) and insulin from medication (exogenous).

What C-peptide level is related to diabetes?

A C-peptide test can’t determine whether a person has diabetes. It merely indicates if your body has the ability to make its own insulin from your pancreas.

To diagnose diabetes, a healthcare provider must order a blood glucose test or glycosylated hemoglobin test (A1c).

Why is a C-peptide test performed?

Your healthcare provider might recommend a C-peptide test for several reasons:

  • Help classify the type of diabetes you have: Type 1 or Type 2 (in conjunction with other tests).
  • Confirm that diabetes treatments are working effectively.
  • Determine whether you’re producing enough insulin or whether you need insulin injections or a pump.
  • Investigate the causes of low blood sugar (hypoglycemia).

  • Rarely, check the status of pancreatic cancer.

Who performs a C-peptide test?

A C-peptide test is usually ordered by a primary care provider or endocrinologist (specialist in glands and hormones). The test is often performed by a nurse or lab technician.

Test Details

How do you test for C-peptide?

A C-peptide test usually requires a blood sample. In an office, clinic or lab, a healthcare provider will insert a thin needle into a vein in your arm. The needle will collect a small amount of blood into a tube. You might feel a slight sting when the needle goes in.

The test also can be conducted on a urine sample. Often, the test requires all urine from a 24-hour period. Your healthcare provider will give you a container, as well as instructions on collecting and returning the sample. You may have to keep the container in a cold place such as a refrigerator or cooler. Providers order this test less often.

Do I need to fast for a C-peptide test?

The blood test may require you to fast (avoid food and drink, other than water) for eight to 12 hours. Sometimes, your healthcare provider might want to test C-peptide after you eat something to stimulate the pancreas. The C-peptide is usually done alongside a glucose level.

Results of a C-peptide test can take one to five days, depending on the healthcare provider and lab.

What is a normal result for a C-peptide test?

A normal result of a C-peptide test ranges from 0.5 ng/mL to 2.0 ng/mL (or 0.17 to 0.83 nmol/L). These values may differ slightly from lab to lab.

What do C-peptide test results mean?

A low level of C-peptide may be considered normal if your blood sugar is low and you haven’t eaten recently. It can also mean that your body isn’t making enough insulin or that you’re taking insulin injections. This can be related to one of the following medical conditions, especially if you have high blood sugar:

  • Type 1 diabetes.
  • Type 2 diabetes (if your pancreas is no longer producing insulin).

Low C-peptide also might indicate that diabetes treatment isn’t working well enough.

Other medical conditions that can cause low C-peptide with low blood sugar levels include:

  • Addison disease, a disorder in which your adrenal glands make too little of the hormones cortisol and aldosterone. This is usually accompanied with a low blood sugar result.
  • Liver disease.
  • Insulin administration by injection/inhalation (exogenous insulin).

A high level of C-peptide might mean your body is making too much insulin. That can point to one of the following medical conditions:

If the test reveals no C-peptide, then you need insulin replacement (usually in the form of insulin injections or possibly, an insulin pump).

Despite central government efforts for a unified system, many beneficiaries in Goa still use designated locations. The Department of Civil Supplies is launching awareness campaigns to educate the public about the benefits of this system, allowing individuals to collect rations in different locations, and reducing the need to return to their registered area.

Ayushman cards can be downloaded through phone applications

Eligible candidates can now download Ayushman cards through mobile applications by downloading the application from the Google Play Store. To download their Ayushman cards from the app beneficiaries can enter their Aadhar number and mobile number and upload recent photographs. Eligible candidates and households with eligible ration cards (only senior citizens 60 years and above) can avail themselves of these cards. Under the Ayushman scheme, one can avail themselves of Rs.5 lakh health insurance covering free medical treatment for 2,250 diseases at 306 government and private hospitals in the district.

One Nation, One Ration Card: Delhi contributes 60.49 lakh transactions to over 1.14 billion total

The ‘One Nation One Ration Card’ (ONORC) Scheme was launched by the Department of Food and Public Distribution on 9 August 2019, as a pilot project in four states, to empower around 81 Crore beneficiaries nationwide, including migrant populations. Delhi has emerged as a leader in the ONORC initiative, achieving a remarkable 60.49 lakh inter-state ration card portability transactions. This achievement coincides with the scheme’s completion of four years, highlighting its significance. As of 4 August 2023, the ONORC scheme has facilitated a total of 1,14,19,47,394 inter and intra-state portability transactions, with intra-state transactions numbering 1,13,31,89,756. Delhi’s success in the ONORC scheme is attributed to e-PoS devices that simplify ration distribution via biometric authentication, benefiting both intra and inter-state migrant beneficiaries. The ONORC ensures migrants access entitled rations at their destination while securing their families’ shares at home. Beneficiaries can obtain food from any Fair Price Shop using existing ration cards and ePoS authentication, reducing leakages. Delhi’s food card holders receive 5 kg/month, with Antyodaya Anna Yojana card holders getting extra sugar. Technology-driven ONORC empowers beneficiaries and enhances efficient, leakage-free essential distribution.

Punjab seeks to include depot owners in doorstep delivery despite their opposition to the Ghar Ghar Ration Yojana

The Ghar Ghar Ration Yojana was previously placed on hold after being challenged in the Punjab and Haryana High Court, but the Aam Aadmi Party administration plans to revive it after making certain amendments to the programme. As part of the strategy, the government now intends to include owners of fair pricing depots and require them to distribute rations. 

Earlier, the state’s depot holders’ welfare organisation had opposed the government’s decision to replace the fair price shops with newly appointed distribution delivery firms that would bring rations to households’ doorsteps. 

According to the National Food Security Act of 2013, the state government was asked to distribute the supply through depot holders in the high court’s ruling on the case. It further contended that, in accordance with periodic orders from the federal and state governments, the only way to distribute necessities is through fair price stores.

Indian residents urged to link Aadhar with ration card by the end of the September

By September 30, 2023, residents must link their ration cards to their Aadhaar cards in order to avoid having their ration cards deleted from the database, according to a public plea from the Department of Civil Supplies and Consumer Affairs. The deadline for Aadhaar enrolment/application has been extended by the Ministry of Consumer Affairs, Food and Public Distribution, Department of Food and Public Distribution, New Delhi, until 30 September 2023.

Additional 10 kg of Ration Per Month for Priority Households in Jammu and Kashmir

The Jammu and Kashmir government has announced that priority households in the Union territory will receive an additional 10 kg of ration per month at subsidised rates. This decision will benefit over 57 lakh individuals. Lieutenant Governor Manoj Sinha made this announcement during a press conference at Raj Bhavan. 

The additional ration will be provided to priority households in the UT under the Prime Minister’s Food Supplementation Scheme. Under this scheme, an additional 10 kg of rice will be provided every month to priority households at subsidised rates, in addition to the Pradhan Mantri Garib Kalyan Anna Yojana. There are approximately 14.32 lakh ration card holders in the priority household’s category, with 57.24 lakh beneficiaries. The maximum ration limit per family per month will be 35 kg in both categories.

Deadline extended for linking Ration and Aadhar card

The last date for linking Aadhar card with Ration card has been extended to 30 September 2023 from 30 June 2023. Ration card is vital documents that allows Indian citizens to avail themselves of gasoline and food grains at subsidised rate. Government prohibits generation of duplicate ration card by linking the beneficiaries’ Aadhar card with their ration card.

Free Ration Received by 85% Target Population in June: Delhi Government

According to the Delhi government, on 14 June, the National Food Security Act (NFSA) ensured that 85% of the target population received free ration in June. During a meeting chaired by Imran Hussain, the Delhi Minister of Food and Civil Supplies, with the Commissioner (food and supplies), it was emphasized that the department should intensify its efforts to consistently provide free rations to eligible individuals in a transparent manner.  

Hussain stated that the government is dedicated to promptly and efficiently addressing public grievances related to the public distribution system. The meeting was organized to evaluate the distribution of free ration under the National Food Security Act and to assess the establishment of vigilance committees at Fair Price Shops across the city.

40 lakh Beneficiaries to Receive National Food Security Act Card by Assam Government in July

In July of this year, the Assam government decided to distribute the National Food Security Act (NFSA) cards to 40 lakh beneficiaries in the state. This decision was made during a state cabinet meeting held at Janata Bhawan in Guwahati on 31 May, chaired by Chief Minister Dr Himanta Biswa Sarma. According to Assam Tourism Minister Jayanta Malla Baruah, who announced the cabinet decisions, the state cabinet has also resolved to establish the Adivasi Welfare & Development Council in accordance with the Adivasi Accord signed on 15 September 2022. The aim of forming the council is to address the political, economic, and educational aspirations of the Adivasi community. It aims to safeguard, preserve, and promote their social, cultural, linguistic, and ethnic identities, as well as ensure the accelerated and focused development of Adivasi-inhabited villages/areas and tea garden areas.

Fortified rice will be provided free of cost to new ration card holders from 1 June

Fortified rice will be provided free of cost by the Uttarakhand Government from 1 June 2023. The government has decided to provide fortified rice apart from other good items, including wheat and rice. The initiative has been taken as the World Health Organisation (WHO) has published a report which mentions that common people are falling prey to several diseases due to the lack of nutrition because of a shortage of quality food.

Ration Card holders in Chhattisgarh to get their ration card linked with Aadhaar by 30 June

The state government of Chhattisgarh has given 30 June to be the last date for all the ration card holders to link their ration card with their Aadhaar. A notification has already been issued by the the Department of Food, Civil Supplies, and Consumer Protection. 

As per the letter issued by the Ministry of Consumer Affairs, Food, and Public Distribution of the Government of India, ration card holders must link their Aadhaar with their ration card to continue receiving the ration supplies via the public distribution system.

Food and Civil Supplies Minister proposes distribution of subsidised salt and sugar

The Food and Civil Supplies Minister Rekha Arya, on Wednesday, stated that the department will propose a scheme for the distribution of sugar and salt for Priority Household (PHH) ration card holders and Antyodaya Anna Yojana (AAY) ration card holders. The ration card holders under AYY and PHH will get two kilograms of sugar and one kilogram of salt every month under the proposed scheme. The officers concerned have been assigned the task of drafting a proposal by the minister so that it can be forwarded to the government. The draft is expected to be presented in the upcoming cabinet meeting. Other than the proposed scheme, several issues like problems faced by ration dealers, non-functional biometric machines at ration shops, etc. Were discussed in the meeting. For payment of dividends to ration dealers under the National Food Security Act, (NFSA) 2013, the minister has added that the funds will be allocated by the state government as soon as it receives budget for the same from Central government.
